CARE for special needs youngsters in Hampshire is set to change from September 1.

A new special educational needs and disability (SEND) code of practice will make a number of changes to the way children’s difficulties are identified and managed.

There will be a new Hampshire Local Offer website that will provide details of all the education, health, social care, leisure services and support available locally for children and young people with special educational needs or disabilities.

Special educational needs statements and post-16 section 139a Moving on Plans (learning difficulty assessments) will gradually be replaced with new, joined up education, health and care plans for children and young people, from birth to 25 years.

Schools will use a new system for tracking the progress of children who need extra help.

SEN Support will replace School Action and School Action Plus and will focus on analysing the effect specialist support has on the youngsters.
