AN investigation has been launched into whether an employee at Salisbury Hospital incorrectly inserted a nasal feeding tube into a patient.
The incident has been reported as a "never event" — a mistake considered so serious it should never happen.
Now the hospital will look into whether the tube was inserted incorrectly or whether it was dislodged by the patient.
For patient confidentiality reasons, the hospital was unable to release any more information about the investigation.
A spokesman for Salisbury NHS Foundation Trust said: "The Trust can confirm that it has reported a never event involving a misplaced naso gastric tube which took place in August 2015.
"The Trust told the family and apologised and is now looking into this as part of its normal procedures."
The could say whether the patient was harmed.
